You’ll choose the applicable state, provide the requested student, parent or agent, school, and document-preference information, review your responses, and complete payment through the online form. What information should I have ready?

Before starting, it helps to have the student’s basic information, current address and date of birth, contact information for the people the student plans to designate, driver’s license information if applicable, and school or university information.

What does “agent” mean?

An agent is a person the student names to act on their behalf for the purposes described in a particular legal document. The authority granted to that person depends on the document and the choices made by the student.

What does “incapacitated” mean?

In general, incapacitated means a person is unable to understand and make their own decisions or communicate their wishes. The precise legal definition and the process for determining incapacity can vary by state and by document.

How will I receive my documents?

After your intake is submitted, your information will be used to prepare your complete document package. You’ll receive the prepared documents along with instructions explaining the remaining steps for review, signing, witnessing, notarization where required, and storage.

Do all of the documents need to be notarized?

No. Completion requirements vary by document and by state. Some documents may require only the student’s signature, some may require witnesses, some may require notarization, and some may allow either witnessing or notarization. Read the instructions provided with each document carefully before signing.

Should I sign the documents before going to a witness or notary?

No. If a document requires a witness or notary, sign it in their presence rather than signing it beforehand. The witness or notary must be able to confirm the signing or verify the signer’s identity.

Where can I find a notary?

Notary services may be available through banks, shipping or office-service stores, college student services offices, libraries, and other local providers. Remote or online notarization may also be available where permitted. Bring appropriate identification with you.

What should I do with the documents after they are signed?

Keep copies of the fully completed documents in a safe but accessible location and provide copies to the people named as agents so they can access them if needed. Depending on the situation, a hospital, bank, school, or other organization may also request a copy before recognizing the authority granted by a document.

How often should the documents be reviewed or updated?

Review the documents every one to three years and whenever a major change occurs, such as moving to another state, changing schools, changing an agent, or changing your wishes. If information or choices need to be changed, new documents may need to be completed rather than editing an already signed document.

What if I notice incorrect information after submitting the form?
